A 60-year-old man, Mr. H, presented to his primary care doctor with multiple vague symptoms in September 2013.

He had been having allergies, sinus congestion, intermittent fever, rash on his legs, and fatigue.

He was diagnosed with acute sinusitis and a rash, and started on antibiotics, nasal spray, and an ointment for his skin.
