A 46-year-old man presented to the ED with back pain, bilateral leg numbness, saddle anesthesia, and right leg weakness.

The ED physician was concerned for spinal cord pathology.

The hospital did not have a neurosurgeon or easily-accessible MRI, so the patient was transferred to another hospital.
