A 56-year-old woman was diagnosed with sinusitis by an NP after 2 days of sinus pain.
She was started on Augmentin.
A few days later, her primary care physician switched her to azithromycin.
A few days after than, her PCP switched to levofloxacin and the patient completed a 10 day course.
She ended up suffering a known complication from levofloxacin (specifically addressed by a black box warning).
The complication was missed by both her PCP and at an ED visit.

I think it’s one of the best plaintiff expert reports I’ve seen… the criticisms are backed by guidelines and the expert does not resort to hyperbole.
