Acute arachnoiditis can also be caused by high-concentration chlorhexidine injection into the epidural space. https://pubmed.ncbi.nlm.nih.gov/30447665/

Wow, very sad case. I've found a few malpractice cases related to either using or not using chlorhexidine.

i'm not aware of local anesthetic toxicity from lidocaine at that relatively low rate, that would cause paraplegia? even if intrathecal? something is missing in this case.
